Our Clients

CCG clients have wide-ranging needs. Some require assistance with cooking, cleaning, doing laundry, and traveling around town. Others need help walking, using the restroom, bathing, grooming, and remembering to take medication. Whatever form of care is required, CCG is committed to safely preserving your loved one's health and independence at home.

Our Process

Each care relationship begins with a visit from an experienced Registered Nurse (RN) who conducts a complementary assessment of the client’s needs and creates a custom Plan of Care (POC) to inform the Home Health Aide (HHA) care team.  Every POC includes a comprehensive review of the client’s medical history and support requirements, as well as detailed information about household and personal preferences. Our RN team helps families honestly assess the client’s ability to stay at home, makes recommendations for home safety modifications and medical support equipment, and outlines the full range of services we provide as a team.

As care is being provided, our team stays closely connected through CCG’s cloud-based communication system to monitor changes in the client’s health.  If our clients experience any changes in their health, our care team is available 24/7 to conduct supplemental evaluations and make adjustments to the POC as needed.

Our Caregiving Team

Our Registered Nurse (RN) team closely manages and monitors our Home Health Aide (HHA) team.  HHA is a general term that encapsulates several designations of caregiver.  Companion, Personal Care Aide (PCA) and Certified Nurse Aide (CNA) are classifications of caregivers that signify various levels of training, and CCG's management team ensures that our clients receive caregivers with the requisite training to accommodate their specific needs.

Each client is carefully partnered with a small team of HHAs based on the client's scope of need, personality type, and common interests. HHAs provide care services according to the case-sensitive POC developed by the assigned RN, and record detailed notes to ensure vigilant monitoring of their clients at all times.

CCG puts prospective caregivers through a rigorous criminal background check, driver’s record check, interview process, and internal training to ensure our clients receive trustworthy and dignifying supervision.  Additionally, each HHA on the CCG team is covered by substantial liability insurance and bonding.
